After you have reviewed our FAQs, please download your Wedding Itinerary and Song Selection Forms.
This can be done very easily by completing 3 simple steps.
1) Download your DJ Paperwork here and save it to your PC or Device. This form will be in pdf format. If you have booked a Photographer with us as well, that page is here.
2) Go to PDFEscape.com and upload your pdf. You will then be able to edit and type on your document. After you have completed your form, save the completed document (pdf) to your PC or Device.
3) Email your completed forms to cherishedmemoriesdj@yahoo.com
In order for us to provide the best possible performance please note that your forms are due back no later than 10 Days Before Your Event. If you are need more time or are booking last minute, contact us to make arrangements with your DJ to get your music to us in a timely manner.
